Connecting People to Community-Based Care

It can be hard for people to access the services and resources they need to thrive. We’re working to change that. Welcome to the HealthierHere Community Hub.

The Community Hub matches individuals and families in King County with community based workers (CBW) who help them find and connect to resources, navigate systems, and champion their needs.

The Community Hub coordinates referrals across King County, bringing together behavioral health, community-based, and healthcare organizations. As the backbone entity of the HealthierHere Community Care Network, it provides centralized infrastructure, administrative functions, training, technical assistance, and support for the community-based workforce so our partners can focus on what they do best.

Trusted Community Connections

Steps in our process:

The HealthierHere Community Hub leverages the power of person-to-person connection and support to help community members navigate complex health and social service systems.

When organizations or individuals refer into the Hub, we match them with a Community Based Worker (CBW) at one of our case management partner organizations.

CBWs play a critical role in the well-being of their communities. Drawing on their shared lived experiences, languages*, and culture, CBWs serve as trusted guides and advocates for their clients’ needs and goals.

Together, this community-based workforce helps reduce barriers and duplication of services, and improve health outcomes and quality of care.

*The HealthierHere Community Hub’s CBWs speak the following languages:
Amharic, Arabic, ASL, Azeri, Bantu Languages (Kikuyu, Kikamba, Kiembu, Shona, Zulu), Bemba, Bengali, Cambodian, Chinese (Mandarin and Cantonese), Chuukese, Dari, English, Farsi, French, Hindi, Korean, Lingala, Marshallese, Ndebele, Nyanja, Oromo, Pashto, Portuguese, Russian, Samoan, Somali, Spanish, Sudanese, Swahili, Tagalog, Tigrinya, Turkish, Ukrainian, Vietnamese, and Wolof.
