The HealthierHere Community Hub will be managing the Care Connect Washington program for King County, WA.
If you or someone you serve tests positive for COVID and need food or other support during isolation/quarantine, please call 1-800-525-0127 then press # to complete an intake for the program. Language assistance is available.
The hotline hours are:
6 a.m. to 10 p.m., Monday
6 a.m. to 6 pm., Tuesday through Sunday and observed state holidays
About the program
- Have you learned that a client or community member has tested positive for COVID?
- Do they and their household need support to isolate safely?
- Call Care Connect at 1-800-525-0127, then press #
- The community member who has tested positive needs to call directly (self-referrals)
Benefits include: a one-time fresh food order delivered to their home, personal care kits, nonperishable food kits, and financial assistance for paying bills (up to $1,500)
When isolation or quarantine ends, the care coordinator can connect people to longer-term local services to support ongoing health and social needs. Care Connect Washington provides the information people need in their preferred language.
Each region works with community-based partners to connect people to services they are eligible for, such as medication delivery, health care, help applying for unemployment, local housing agencies, food banks, childcare providers and more.
