Partnering organizations need to register their organization by indicating selection of participating project(s) and financial information so that HealthierHere can distribute earned incentive payments if you are selected as a partnering provider.

In order to gauge the capacity and readiness of potential partners for systems change, we require that all potential partners complete a Current State Assessment. Assessment results will be used to inform our investment plans and project implementation plans, the development of change plans for partnering organizations, and ultimately, contribute to system-wide health care transformation. We will keep the information you provide confidential and only share de-identified, aggregate data.

This assessment of health care and related support organizations in King County will help HealthierHere and its partners develop an understanding of the current state in the areas of health information exchange (HIE) and health information technology (HIT). We want to understand the elements of HIE and HIT your organization believes are important now and in the coming 12-18 months to address fragmentation of health and social services through increasing care coordination, population health management, and outcome focused care for individuals with complex health and social needs.

Assessment results will be used to inform HealthierHere investment plans and project implementation plans, the development of change plans for partnering provider organizations, and ultimately, contribute to system-wide health care transformation

  • What an Accountable Community of Health is and how it is different from a grant program
  • Which types of programs and health outcomes are targets for innovation
  • Opportunities to partner, learn, network, and participate in ACH innovations
  • How we intend to bring funding to King County for system-wide investment
  • Ways your community can help shape priorities and decisions
