The HealthierHere Community Hub matches individuals with culturally and linguistically appropriate Care Coordinators from our Hub network to provide Care Connect Washington services. Eligibility is open to all King County residents who have been impacted by the COVID-19 pandemic and are looking for care coordination support. Community members can call 1-833-453-0336 to complete an intake for the program. Language assistance is available.
