At HealthierHere, we remain steadfast in our commitment to a simple but powerful promise: that every person in King County deserves equitable access to whole-person care—care that supports not just physical health, but mental, emotional, and social well-being—that helps people thrive.

Since our founding in 2017, we’ve worked across sectors to dismantle systemic barriers and co-design solutions that reflect the lived experiences of our communities. In 2024, that vision took meaningful steps forward. Together, we:

  • Navigated the transition to Medicaid Transformation Project 2.0, embracing new opportunities to innovate and collaborate.
  • Launched programs and technologies that empower our partners and uplift communities.
  • Establish our 2024–2028 Strategic Priorities to guide the next phase of our work.

These milestones are more than achievements—they are lifelines in a time of growing uncertainty. The environment we’re operating in is increasingly complex: economic pressures, workforce shortages, and persistent inequities continue to challenge our systems and communities. Yet, it is precisely in this moment that our work matters most.

We are building a foundation for a countywide—and ultimately statewide—system of whole-person care coordination that is more responsive, more equitable, and more sustainable. This is not easy work, but it is essential. And we are making progress, step by step, because of your partnership.

Highlights

Alongside our network of partners, we’re working together to ensure all individuals can access the care and resources they need to live well. As the Accountable Community of Health (ACH) for King County, HealthierHere is part of a statewide effort working to transform health and social care systems. Below, see highlights from our collaborative work in 2024.
The HealthierHere Community Hub is a free, confidential service that helps King County residents connect with the resources they need — from food, to healthcare, utility assistance, and more. Built for long-term impact and sustainability, the Community Hub onboarded 17 community-based organizations in 2024 to form a coordinated network of support, with community based workers (CBWs) at the center. They bring deep knowledge of local resources and communities, and a commitment to walking alongside clients as long as support is needed.

Connect2 is the care coordination technology co-designed by HealthierHere alongside community partners. Our suite of technology solutions designed for community-based workers and organizations coordinating services help to ensure people get connected to the whole-person care they need.

At HealthierHere, we believe lasting change begins with connection. Our 2024 Cultivating Connections series brought partners and community voices together to share stories, learn from one another, and spark ideas for a healthier King County. These gatherings deepened trust and strengthened the relationships that make collective action possible.

Bringing together community, tribal, native-led/native-serving, healthcare, behavioral health, and social service organizations, the HealthierHere Community Care Network strengthens cross-sector collaboration through partnership, supports the community-based workforce, and fosters shared learning to advance health equity and improve outcomes for people across King County.

HealthierHere’s Community and Tribal Innovations initiative supported 14 partner organizations to develop inventive community-based solutions to transform the way that people access or experience health and social services in King County. Partners created Scale & Sustain Playbooks to help others discover practical insights.
Real change begins with listening. In 2024, HealthierHere’s Landscape Assessment invited community members and partners across King County to share their challenges and hopes for a healthier future. Their voices now guide how we work together to create a more equitable system of care.

In 2024, we nearly doubled our staff, developed an internal culture charter, and celebrated the launch and growth of multiple programs many years in the works.

2024 has shown us what’s possible when communities lead and systems listen. With the strength of our partnerships, HealthierHere is building momentum toward a more connected, equitable King County. We’re proud of the groundwork we’ve laid together, and are even more excited for the work ahead.
