Position Summary
HealthierHere is a nonprofit regional collaborative committed to reducing health disparities, advancing equity and improving the well-being of all people in King County through innovative, cross-sector collaborations. Through partnership with community members and leaders from diverse sectors (health providers, social service agencies, community organization, government agencies, etc.), HealthierHere identified a portfolio of initiatives aimed at improving the experience and outcomes of providers and community members across King County. This work includes improving and increasing access to whole-person, integrated care in King County, and a key priority has been to strengthen partnerships between clinical providers and community-based organizations addressing social determinants of health (i.e., housing, transportation, food security, employment, etc.). Despite the multiple care coordination programs and initiatives in King County, HealthierHere’s partners repeatedly cite ongoing fragmentation, lapses in communication, and lack of interoperable technologies that persist in and among the many forms of care coordination that individuals experience in King County. HealthierHere is working to improve the Community Based Care Coordination (CBCC) system through collaborative co-design and transformation initiatives.
The Connect2 Community Network is working to reduce these challenges and connect community members with the support they need to thrive and be healthy. This network aims to strengthen the coordination of care by connecting people, service organizations, and healthcare, community, and Tribal partners more quickly and effectively – resulting in a healthier, more equitable community for all. The Product Manager reports directly to the Director of Community Information Exchange and Care Coordination. This position focuses on planning, developing, launching, and improving the Connect2 Community Network, a Community Information Exchange, as a service. This position is responsible for overseeing all activities related to researching user requirements, designing and managing the roadmap for, and promoting the features and functions of the Connect2 Community Network to deliver value to our community, clinical, and Tribal partners. This person must be able to research and understand the needs of clients and users, and work with leadership to facilitate prioritization and translate into system integrations, features, functions, and enhancements over time. This person will also oversee and coordinate contractors and vendors building out platform capabilities including the contracted IT platform vendor provided SaaS managed services, to strengthen the Connect2 Community Network.
The ideal candidate enjoys developing strategies and solutions to complex problems with input from a diverse pool of partners/organizations who represent a wide range of experience and capacity providing services that address whole person health, including partners providing social services to address the social determinants of health. They enjoy technical and adaptive challenges as well as innovative problem solving - doing what has never been done before. They bring a passion for improving health equity by enabling health care, Tribal and community organizations to use information technology in a way that quickly matches community members to needed services while protecting client privacy.

Major Responsibilities
Support the development, implementation, and growth of the Connect2 Community Network and Exchange.
- Collaborate with team members to co-design and facilitate community-led processes to engage community, tribal, and clinical partners to capture requirements, co-design solutions, and implement capabilities that will meet partner needs.
- Identify value for clients, users, and funders; facilitate developing and implementing use cases based on partners’ identified client, organizational, and workflow needs to optimize value.
- Ensure technology and partner support are aligned with community prioritized use cases and needs (in partnership with Chief Technology Officer).
- Track advancements in relevant technologies and standards to support new functionality and enhancements.
- Collaborate with team members to improve training, onboarding, and on-going learning opportunities for Network partners to optimize adoption and utilization of the CIE.
Lead and manage multiple vendor and consultant contracts.
- Oversee selected contractors in collaboration with the Director of CIE and Care Coordination and the contracted Chief Technology Officer.
- Coordinate and review project progress by creating a product roadmap, tracking planned activities against the roadmap, resolving problems and recommending actions to respond to emerging issues or changing needs to the Director of CIE and Care Coordination.
Ensure alignment with HealthierHere initiatives, investments, and values.
- Support HealthierHere in meeting all reporting requirements and all project specific deliverables associated with advancing Connect2 Community Network and Exchange and Care Coordination.
- Collaborate with the Director of CIE and Care Coordination and the Development Director to identify and pursue pathways to financial sustainability with support from Connect2 Advisory Group.
- Support writing and management of funding proposals and/ or contracts, as needed.
- Build effective working relationships with peers, leadership, and partners.
- Apply HealthierHere’s equity and inclusion principles to all assignments; promote outreach, inclusion, and active community engagement across a wide array of organizational partners.
- Perform other duties as assigned.
Experience, Qualifications, Knowledge, Skills
- At least 5 years’ experience working with clinical and/or community partners on health-related programs that focus on new and enhanced services.
- Experience building and managing product roadmaps that meet stakeholder requirements and priorities.
- Experience synthesizing requirements and challenges from diverse stakeholders and transforming input into scalable solutions.
- Experience building and implementing emerging information technology services and system integrations.
- Skilled project management, specifically the ability to carry out complex work in collaboration with colleagues and partners, leading projects to completion with limited supervision.
- Strong communicator, both verbal and written, with the ability to build trust while mobilizing people to take action.
- Demonstrated problem solver; ability to identify partner “pain points” and collaboratively develop solutions to address them; “customer success” experience a plus.
- Demonstrated ability to respectfully engage individuals representing different cultures and backgrounds with ease, adapting language and style to the context both in person and remotely.
- Experience supporting clients and/or partners while ensuring they meet shared community standards.
- Demonstrated skill in translating advanced technical concepts into a format and language that is understandable and actionable to a broad audience of stakeholders.
- Demonstrated ability to navigate and adapt to fast-paced, changing internal and external contexts.
- Proactively sees organizational gaps and potential issues; proposes solutions.
- Deep and demonstrated personal commitment to advancing equity and social justice, and strong alignment with HealthierHere’s organizational values.
- Proficiency with Microsoft Office (Outlook, Word, Excel, PowerPoint, and SharePoint) and Adobe (Acrobat DC).
Desired Qualifications
The most competitive candidates will lead with a passion for equity and bring:
- Experience working on issues related to health equity, health and wellness, social determinants of health and/or social services at a non-profit organization or community coalition.
- Familiarity with King County and knowledge of social, clinical, and/or tribal services in the region.
- Experience providing social or clinical services and/or navigation services in King County.
- Experience synthesizing and interpreting data and leading quality or performance improvement initiatives.
- Experience designing and implementing new initiatives and system transformation.

Schedule and Worksite
This position works full-time Monday – Friday during standard business hours. HealthierHere is currently performing all work remotely. Some of the work may continue remotely on an ongoing basis, depending on business needs. There may be situations when the employee is required to attend work related meetings and functions in person. Employees will be provided with a HealthierHere-issued laptop and must maintain a workspace with an internet connection where they can reliably perform work and remain available and responsive during scheduled work hours.
