Semiannual Progress Report - Fall 2019 Edition

HealthierHere kicked off the implementation phase of its work in 2019. Together with our partners, we began to strategically strengthen infrastructure and capacity, build new partnerships, develop skills, share knowledge, collaborate on system wide tools, and spark innovation all with a shared commitment to improve health disparities and advance equity for the people of our region. To support this work, HealthierHere also distributed $1.8 million to clinical and community partners in the first half of 2019.

Lead with Equity

We teamed up with our first round of Community Grants Program Partners to summarize the data they collected through community engagement activities:

‍908 Surveys - 5 Focus Groups

Together, we developed an easy-to-read format that is now available as a poster in 14 languages. The data is being used to further community conversations and inform efforts to improve health care in our region.

Governing Board member Elizabeth Tail opened our Building Bridges event with a thoughtful and respectful land acknowledgment.

Our Equity and Engagement Team facilitated the first meeting of a Committee on Native and Indigenous Health in King County. Representatives from Tribes and Native and Indigenous Community-based Organizations joined in conversation and voiced support for continuing this effort.

Catalyze and Test Innovations

We awarded our first partner-driven Tests of Innovation:

  • Country Doctor Community Health Centers’ Innovative Approach to Transforming Medication Assisted Treatment in Central Seattle
  • Public Health – Seattle & King County’s Buprenorphine at Navos Mental Health and Wellness Center Project (Bupe NoW) Expansion in South King County
  • Seattle Fire Department’s Enhancing the Capacity of a Fire-Based Mobile Integrated Health System in Central Seattle

Congratulations and thanks to these organizations for helping lead the way to innovative ways of delivering care that, if effective, can be scaled across our region.


Co-design Systemwide Tools

We convened cross-sector events to help organizations build new partnerships, learn from one another, and design tools to improve care:

  • 20 Learning Collaborative sessions attended by Behavioral Health Agencies (BHAs), federally qualified health centers (FQHCs), hospitals and managed care organizations
  • Building Bridges, a full day event with leading experts on cross sector partnership, Dr. Pritpal Tamber and Lori Peterson, joined by 200 participants from more than 80 clinical and community-based organizations
  • First Co-Design Collaborative attended by nearly 100 representatives from over 50 community and clinical organizations

We engaged cross-sector stakeholders in a monthly workgroup to co-design a Shared Care Plan pilot, set to launch in late 2019.

We explored the potential for a regional Community Information Exchange (CIE). In collaboration with local stakeholders, including several organizations currently using CIE-like networks, we gauged interest, developed a shared vision, and explored first steps.

Strengthen Foundation Infrastructure

We hosted 5 Webinars to share expertise on:

  • Transportation
  • Harm Reduction
  • Overdose Notifications
  • Improving Care for Multi-Visit Patients
  • Substance Use Disorder Consent Management

We enrolled 19 BHAs in a 10-month Value-Based Payment Academy training and coaching program.

We helped clinical partners build capacity for integration through Training, Technical Assistance and Coaching in:

Integrated Managed Care: 11 Organizations

Collective Ambulatory*: 34 Organizations
