HealthierHere is proud to share our newly revised clinical summaries. We created the clinical summaries to communicate the depth and breadth of the HealthierHere portfolio. The 11 clinical summaries represent distinct sub-project areas of HealthierHere’s portfolio of four HCA-designated projects. They were developed in collaboration with our stakeholders, including many subject matter experts who put in extra hours of review and feedback for which we are very grateful. These clinical summaries will drive our future transformation work, and we encourage you to review them to get a better sense of the focus populations and key elements for system transformation in these 11 areas.
HealthierHere is now accepting applications for the position of Community and Tribal Engagement Manager. The Community and Tribal Engagement Manager will report directly to the Director of Equity and Community Partnerships and is responsible for assisting with the design and implementation of HealthierHere’s overall engagement strategy in support of HealthierHere’s Project Portfolio. This will include, but not be limited to, ensuring strong connectivity and communication with community members, community-based organizations (CBOs), coalitions, consumers of publicly funded services, stakeholder agencies, and tribal partners. Specifically, s/he will support the strategic coordination of HealthierHere’s community and tribal engagement activities, including building partnerships with and supporting CBOs that authentically engage diverse communities and Medicaid client populations.
